The State Independent Living Program and Centers for Independent Living are required to submit annual reports to our federal counterparts.  This report, known as the "704 Report" contains information on the funds and activities of the IL program during the reporting year.

In an effort to increase transparency and ensure that consumers are empowered to participate in the development and implementation of IL services, the SILC is now offering its 704 Reports online.
